Although your insurance company should provide a Proof of Loss Form immediately, do not wait for them to provide one – make a request for a form from your insurance company when you give your notice of loss. You must provide the completed Proof of Loss to the insurance company within the specified time set out in your insurance contract.

The proof of loss form will most likely request evidence to support any amounts claimed as losses and your interest in the property for which you seek recovery such as receipts, titles, registrations, etc.
